Fighters and Trainers

Have your FULL PHYSICAL completed before the event by your own Doctor or a Medical Clinic.

Please make sure of the following requirements so your physical does not get denied:

• Make sure the form is filled out in full, front and back.
  • IF NOT: You will be required to pay $40 to have a correct and complete form done at weigh-ins
• Make sure your physical is done by a MD or DO only
  • IF NOT: You will be required to pay $40 to have a correct and complete form done at weigh-ins
• Make sure the blow section (at bottom of page 2) is filled out by the MD or DO
  • EXAMINING PHYSICIAN: Based on your personal observation and review of the test results it is your medical opinion that this applicant is physically fit to compete as a
